Opioids and Opiates

What Are Opiates and Opioids?

Although the terms are often used interchangeably, there’s a difference:

Opiates – Naturally derived from the opium poppy plant.

Opioids – A broader category that includes opiates and semi-synthetic or fully synthetic drugs that act on opioid receptors in the brain.

Both are highly addictive and dangerous when misused.

Common Street Opiates

These are directly derived from the opium poppy:

Heroin – The most common illicit opiate; can be injected, snorted, or smoked.

Morphine – Powerful painkiller, sometimes diverted from medical use.

Codeine – Commonly found in prescription cough syrups; often mixed into drinks (“lean,” “purple drank”).

Common Street Opioids

Opioids can be semi-synthetic or fully synthetic:

Semi-synthetic opioids:

Oxycodone (OxyContin®, Percocet®) – Known on the street as “Oxy,” “Percs,” or “Hillbilly Heroin.”

Hydrocodone (Vicodin®, Norco®) – Often called “Vikes” or “Hydros.”

Hydromorphone (Dilaudid®) – Street names include “D” or “Dust.”

Semi-synthetic opioids:

Fentanyl – Extremely potent and often mixed into heroin, cocaine, or counterfeit pills without the user’s knowledge.

Methadone – Used in opioid treatment programs, but sometimes misused.

Tramadol – Less potent, but still addictive and dangerous when abused.

The Dangers of Opiate and Opioid Misuse

Opioids and opiates attach to receptors in the brain that control pain and pleasure, creating a powerful sense of euphoria. Over time, the body develops tolerance, meaning higher and higher doses are needed to achieve the same effect. This quickly leads to dependence and dramatically increases the risk of overdose.

Illicit opioids are especially dangerous because they are often cut with fentanyl or other potent substances without the user’s knowledge. Just one dose can be lethal.
