Levels of care

Flexible, Structured Levels of Care

Every stay starts with an assessment, then moves through whichever of these levels fit, in either direction, as your needs change.

Residential Treatment

Flagship

Our most structured, primary level of care. Clients live on-site with 24/7 supervision, daily therapy, and psychiatric care for both addiction and mental health.

What Residential Treatment Includes

Detox

A safe, clinically monitored start. Our team manages withdrawal around the clock, so your body can safely let go of what it has been depending on before treatment begins in earnest.

See What Medical Detox Involves

PHP, Day Treatment (SUD, MH)

Full clinical days without a residential stay. The same intensity of care as residential treatment, with evenings spent at home or in transitional living.

See How PHP Day Treatment Works

IOP, Intensive Outpatient (SUD, MH)

Structured treatment that flexes around a life you are already rebuilding. Real clinical support without stepping away from work, school, or family.

What IOP Treatment Involves

Transitional Living / Enhanced Sober Living

A stable, substance-free home base as you find your footing. Structure and community support while you build toward full independence.

Outpatient (OP)

The lightest touch level of care, built for ongoing support once you are stable. A natural step down after residential, PHP, or IOP.

Support Beyond Levels of Care

Case Management

One dedicated person who knows your file, your history, and your name for the entire stay, coordinating care across every level so nothing falls through the cracks.

Medication-Assisted Treatment

Medical support available where it is clinically appropriate, always paired with therapy and psychiatric oversight, never offered as treatment on its own.

Family Program

Whether that's the family you were born into, the one you chose, or both, our Family Program doesn't assume blood relationships. It's open to clients in PHP or IOP, with their therapist's approval.

Alumni Program

"Their alumni program is incredible as they are big on following up and supporting former patients even after they leave the facility."

How do I know which level of care is right for me?+

You do not have to figure that out alone. A clinical assessment during intake determines the right starting point, and your team adjusts it as you progress.

Can I move between levels of care as I improve?+

Yes. Most clients move through a sequence, often starting at Detox or Residential and stepping down to PHP, IOP, or Outpatient as they stabilize.

What if I'm not sure I need Residential care?+

That is common, and it is exactly what the assessment call is for. Many people start with a lower level of care and step up only if it is clinically needed.

Is Case Management included automatically, or is it a separate program?+

Case Management runs alongside your primary level of care. It is one dedicated person who knows your file, your history, and your name for the entire stay.

Does insurance cover every level of care the same way?+

Coverage can vary by level of care and by plan. Our admissions team verifies your specific benefits before you commit to anything.

Can I transfer to Breathe from a hospital or another treatment program?+

Yes. A transfer is handled the same way as any admission. You complete an intake, are introduced to the program, and, if you're coming into residential, meet with our nurses first. From there you rest or start joining groups at a pace that fits how you're feeling.

Who is welcome at Breathe?+

Clients of every background. Breathe offers LGBTQIA+ affirming care at every level of treatment, and partners and chosen family are welcome in our family program.

What happens after I complete treatment?+

Our Alumni Program provides ongoing support. Recovery does not end at discharge, and neither does our involvement.

Not sure which level of care is right for you?

Our admissions team talks through your situation first, then matches you to the right starting point. No pressure, no obligation.
